Norwich Escalation Avoidance Team (NEAT) provides Norwich's single point of access for urgent, unplanned health and social care needs where co-located staff work together to coordinate an integrated response which supports and manages patients and carers safely in the community through a period of crises, using the most appropriate level of intervention.

There is a cost associated with this service.

Age Range: 18 and over
Target Groups: Blind / Visual impairment, Carer, Deaf / Hearing impairment, Disability, End of Life / Palliative, Learning Disabilities, Mental Health, NHS and Social Service staff, Older people, Substance Misusers

Referral Procedure: Referral is via a health / social professional only.
How Service is Delivered: NEAT does not deliver services directly but through a multi-agency disciplinary meeting we coordinate a crises plan using existing resources to support patient to avoid unnecessary hospital or residential care. We are a telephone service only but the services we deploy have various methods of service delivery including home visits.
Opening Times: Monday to Friday, 9 a.m - 4.30 p.m.
