The falls service is provided county-wide from our community therapy teams, and provides home-based assessment to identify the factors and risks, which may contribute to a person’s falls.

The aim of the assessment is to develop a personalised action plan to enable the person to improve their confidence, balance, strength and stamina. In reducing the risks, we hope to boost independence and reduce rate and frequency of falls.

The assessment includes: mobility, self-care, bone strength, medications, home environment, medical factors, falls history, vision, hearing, foot health and memory/cognition.

If specific ongoing therapy is required, our teams can refer to physiotherapy or occupational therapy.

Interpretation and translation services can be arranged by staff, for users of this service.

Home visits are provided by this service.
