The service provides care, advice, support, education and medication management to people who have been diagnosed with heart failure. Confirmed by Echocardiogram. Care is offered to those who are house bound as well as those who are able to attend a clinic. The heart failure service works closely with all of the other community health and social services.
Home visits are provided by this service.
Interpretation and translation services can be arranged by staff, for users of this service.

Health professionals in primary, secondary and teritiary care.
After initial referral individuals can refer themselves back to the service if required.

How Service is Delivered:
|
The service is delivered by home visit or clinic visit
|

Opening Times:
|
Monday to Friday 9 a.m - 5 p.m. Pre arranged evening appointments available up until 8pm.
The clinic runs on Friday mornings at Magdalen Medical Practice, Lawson Road, Norwich (not every Friday), appointments to be arranged in advance.
There is an answer phone service giving date of return if heart failure nurse is unavailable for any period.

The Patient Advice and Liaison Service (PALS) can listen to your issues, concerns, compliments or suggestions, resolve problems where possible, and feed back to services to aid service development.
