Family Nurse Partnership Programme is an intensive, preventative home visiting programme for first time teenage parents, 19 years and under at conception. The same family nurse works with families from early pregnancy until the child is two years old.

Interpretation and translation services can be arranged by staff, for users of this service.

Home visits are provided by this service.

Age Range: 19 and under
Target Groups: Children (Pre School), Families, Parent - Teenage, Women
Related Services: Looked After Children (LAC) And Young Person Health Service (Norfolk Community Health & Care NHS Trust)
Referral Procedure: Referrals from Health Professionals, Social Care Professionals and Voluntary Agencies are accepted, as well as self referrals.
Service Delivery: Home or community location if service users prefer.
Opening Times: The family nurses visit clients homes. Home visits are arranged to suit the needs of the client. However, they can be arranged from approximately 8.30 a.m. - 6.30 p.m. There is also an answerphone available for this service outside of normal working hours.
Compliments / Complaints: The Patient Advice and Liaison Service (PALS) can listen to your issues, concerns, compliments or suggestions, resolve problems where possible and feed back to services to aid service development.
