The Community Matrons are Nurses who work directly with patients who have unpredictable health needs connected to long term conditions, such as respiratory or cardiac diseases that may cause frequent but unplanned hospital admissions/attendances. This service is offered within patients own homes / care homes or occasionally within a clinic environment.

The Community Matrons service is provided alongside the Community Nurses and Therapists.

Interpretation and translation services can be arranged by staff, for users of this service.

Home visits are provided by this service.

Referral Procedure: To use this service, you will need to be referred by a health professional. This means you must see your doctor or nurse. They will refer you to the service that is the most suitable for your health needs. The service will then contact you to make an appointment or explain what will happen next.
How Service is Delivered: Home Visits, Telephone or video consultations if appropriate.
Opening Times: Monday to Friday, 8.00 a.m - 5.00 p.m.
Parking and Transport: On site parking with disabled spaces available. Site accessible by public and community transport.
Access and Facilities: Main entrance and toilet facilities are well signposted and wheelchair accessible.
Compliments / Complaints: The Patient Advice and Liaison Service (PALS) can listen to your issues, concerns, compliments or suggestions, resolve problems where possible, and feed back to services to aid service development.
