The Community Matrons work directly with patients who have unpredictable health needs particularly connected to long term conditions, such as respiratory or cardiac diseases that may cause frequent but unnecessary hospital admissions. This service is offered within patients own homes/care homes.

The Community Matrons service is provided as part of the Integrated Care Team

Home visits can be provided by this service.

Interpretation and translation services can be arranged by staff, for users of this service.

Referral Procedure: To use this service, you will need to be referred by a health professional. This means you must see your doctor or nurse. They will send your details to a service. Your doctor or nurse will refer you to the service that is the most suitable for your health needs. The service will then contact you to make an appointment or explain what will happen next.
How Service is Delivered: Patients will only be seen in their home
Opening Times: Monday to Friday 8.30 a.m - 4.30 p.m
Parking and Transport: Onsite parking spaces are available.
Disabled parking spaces are available.

You can get to this service using community transport. For example, volunteer buses or taxis.
Compliments / Complaints: The Patient Advice and Liaison Service (PALS) can listen to your issues, concerns, compliments or suggestions, resolve problems where possible, and feed back to services to aid service development.
