The Community Matron service provides case management for patients aged 18 and over with complex long term conditions. They also try to make sure patients can be cared for at home, preventing unnecessary and avoidable hospital admissions. Home visits are provided by this service.

Referral Procedure: To use this service, you will need to be referred by a health professional. This means you must see your doctor or nurse and they will send your details to the service. Your doctor or nurse will refer you to the service that is the most suitable for your health needs. The service will then contact you to make an appointment or explain what will happen next.
How Service is Delivered: Domiciliary home visits
Opening Times: Seven days a week, 9 a.m - 5 p.m.
Compliments / Complaints: The Patient Advice and Liaison Service (PALS) can listen to your issues, concerns, compliments or suggestions, resolve problems where possible, and feed back to services to aid service development.
