The service provides advice and a comprehensive range of treatments for housebound patients that enable an individual to avoid unnecessary admission to hospital, or where hospitalisation is necessary, to facilitate an early discharge back into their community. Although please note community nurses do support patients with palliative care support visits who may not yet be housebound, and community therapists support patients with their rehabilitation goals who may also not be housebound. The community nursing service also support patients that are not housebound that attend Leg Ulcer Clinics.

The team covers West Norfolk and surrounding villages (Some villages may be covered by a partner team - patients will normally be seen based on which GP Practice they are registered with)
