The service shall provide advice; nursing and therapy care to support patients remaining in their usual place of residence, prevent avoidable hospital admissions, and enable patients to return home from a place of care as soon as possible. The service shall also provide end of life care at the patient’s preferred place for care. Interpretation and translation services can be arranged by staff, for users of this service. Home visits are provided by this service. An information leaflet about the service is available.

How Service is Delivered:	Inpatient's Units, Community Nursing Team, Community Therapy Team, Clinics - outpatients.
Opening Times:	Normal Service: 7 days a week, 8 a.m - 6 p.m. Out of Hours service: 6 p.m - 8 a.m.
Parking and Transport:	Onsite parking available for patients. Disabled access available.
Compliments / Complaints:	The Patient Advice and Liaison Service (PALS) can listen to your issues, concerns, compliments or suggestions, resolve problems where possible, and feed back to services to aid service development.
