This service helps patients over the age of 18, who have diabetes and may need specialist review of their diabetes. Patients can be seen at different venues across West Norfolk or at home if they are housebound and unable to attend a clinic appointment.

Diabetes education sessions for people newly diagnosed with type 2 diabetes are offered at different venues throughout West Norfolk and an online version is offered as an alternative for those that would prefer this method. All patients newly diagnosed with type 2 diabetes will be referred into this programme by their GP surgery.

Type 2 Diabetes Education Programme - West Norfolk (Norfolk Community Health & Care NHS Trust)
Referral Procedure: To use this service, you will need to be referred by a health professional. This means you must see your Doctor, Consultant or Nurse. They will send your details to a service and will refer you to the service that is the most suitable for your health needs. The service will then contact you to make an appointment or explain what will happen next.
How Service is Delivered: Patients are referred in via their GP for review and are seen in clinics or via Video appointment as needed. Home visits can be arranged for patients who are unable to leave their home to attend health appointments.
Opening Times: Monday to Friday, 9.00 a.m - 5.00 p.m
An answerphone is available at other times.
Parking and Transport: Public car parks are nearby and disabled parking spaces are available.

Compliments / Complaints: The Patient Advice and Liaison Service (PALS) can listen to your issues, concerns, compliments or suggestions, resolve problems where possible, and feed back to services to aid service development.
