The Adult Community Speech and Language Therapy Team (central Norfolk) is a small and busy team of fully qualified and registered Speech and Language Therapists and a Speech and Language therapy Assistant. We work within the North and South of Norfolk and greater Norwich area. We offer expert assessment and management of communication and swallowing problems in adults over the age of 18. These could be associated with ageing, stroke, mental health or progressive neurological conditions.
The service will:
- Work with the person and those around them to help them to understand all of their swallowing and communication needs
- Help people to make choices and set goals for improving and coping with their communication and swallowing problems
- Help people of varying abilities to work towards their goals, reducing harm, promoting quality of life and maximising their potential
- Enable people to use equipment and technology to support their communication needs
Wherever possible we ask patients to attend outpatient clinic appointments or participate in appointments via video link. Home visits can be offered for people who are bed bound or those who need to be seen at home as a reasonable adjustment due to intellectual or cognitive impairments. People can also be seen on community hospital inpatient units.
Interpretation and translation services are available to people accessing the service. Support can be accessed via Intran if required.

Referral Procedure:

To use this service, you will need to be referred by a health professional if you are having swallowing difficulties.. This means you must see your doctor, nurse or other healthcare professional and they will send your details to the service OR if your difficulty is solely with communication you could contact the service yourself and give them your details. The service will then contact you to make an appointment or explain what will happen next.

How Service is Delivered:
|
The service will be provided in inpatient and outpatient community hospital clinics, community settings, in care homes and, where necessary, at home. This may be face to face or telehealth (via telephone/video link).
The service will work as part of multi-disciplinary teams (carrying out joint intervention with other members of the MDT as required) and liaise with voluntary groups, such as Speakability, the MND Association, the MS Society and the Parkinson's Disease Society; to ensure the best care and support for our clients.

Monday to Friday, 8.30 a.m - 4.30 p.m. An answerphone is available at other times.

Main entrance and toilet facilities are well sign posted and wheelchair accessible. Loop hearing aid system also available. Easy read documents can be provided where required.

Compliments / Complaints:

The Patient Advice and Liaison Service (PALS) can listen to your issues, concerns, compliments or suggestions, resolve problems where possible, and feed back to services to aid service development.
