This service is for the families of disabled children/young people aged 0-18 years (19 if still in education) living in the area covered by NHS Norfolk who have a range of professionals involved with them from two or more agencies (i.e. health / social care / voluntary / housing etc). The child must have complex health needs or have a high level of need. A key worker is identified in consultation with the family, from the professionals already involved with the child. Sometimes, members of the family or someone close to them may want to take on some of the role. The key worker, with support from the key worker service coordinator, acts as a single point of contact and source of support to ensure that the family and child receive well coordinated services.

Interpretation and translation services can be arranged by staff, for users of this service.

Home visits can be provided by this service.

Opening Times: Monday to Friday, 08:30 a.m - 16:30 p.m. An answerphone is available at other times.
Compliments / Complaints: The Patient Advice and Liaison Service (PALS) can listen to your issues, concerns, compliments or suggestions, resolve problems where possible, and feed back to services to aid service development.
