The Therapy Team provides physiotherapy and occupational therapy assessment and intervention to people within South Norfolk, Breckland and Mid Norfolk with non-complex rehab needs. This is primarily a domiciliary based service.
- Comprehensive holistic assessment to identify the persons broader needs, e.g. health, social, support.
- Provision of treatment, equipment and / or support and guidance to meet the identified needs of the person and their carer.
- Work in collaboration with other providers and services such as Social Services, Community Nursing teams, Supported Care Service, Neurology Specialist Nursing Service.
- Offer facility for self referral to address changes in need / physical condition.
- Offer education and strategies for enabling the person and / or carer to best manage their condition.
- Form firm links between other health professionals and agencies involved in the person's care.
- Our aim is to enable the person to achieve their full physical and functional level at each stage of the disease process.
Home visits are provided by this service.
Interpretation and translation services can be arranged by staff, for users of this service.

Referral Procedure:

Referrals for newly diagnosed patients with a Neurological condition must always be made by a health professional. All other patients can be referred by healthcare staff, social services, carers and self referral.

Monday to Friday, 09:00 - 17:00.

Limited onsite parking available. Accessible by public and community transport.

Main entrance and toilet facilities are well sign posted and wheelchair accessible. A Loop hearing aid system is also available.

Compliments / Complaints:
|
The Patient Advice and Liaison Service (PALS) can listen to your issues, concerns, compliments or suggestions, resolve problems where possible, and feed back to services to aid service development.
