The Type 2 Diabetes Education Programme is for people recently diagnosed with Type 2 Diabetes. It is a group session, delivered by Healthcare professionals held over three and a half hours. Patients are referred onto the Programme by their GP or Practice Nurse.

Age Range: 18 and over

Referral Procedure: Referral is via health professional only.
How Service is Delivered: Face to face group work.
Compliments / Complaints: The Patient Advice and Liaison Service (PALS) can listen to your issues, concerns, compliments or suggestions, resolve problems where possible, and feed back to services to aid service development.
