If you have had heart surgery, a heart attack and/or angioplasty procedure or heart failure, this service will help you start exercising to improve your heart health and provide a comprehensive education program to enable you to identify risk factors and make lifestyle modifications to reduce your risk of further issues.
We offer an eight-week Group Cardiac Rehab Program at five venues around the County including the following:
- Monday - Fakenham Community Centre
- Tuesday - Long Stratton Village Hall
- Wednesday - Aylsham Town Hall
- Thursday - Swaffham Community Centre (Iceni Partnership)
- Friday - Cloverhill Village Hall, Norwich
We offer an initial assessment on week one in the afternoon between 12:30 and 14:00s, including a fitness walk test if appropriate. The Group Program Runs from 9.15-12.00hrs and includes exercise and an education session for six weeks
Education topics include:
- The Heart, how it works, and treatment and surgery
- Risk Factors for Heart Disease
- Wellbeing
- Benefits of Exercise
- Healthy Eating, food labelling and weight management
- Medications for the Heart
- Practical Resuscitation
Week eight is a discharge session, including a repeat fitness test if appropriate, between 12:30-14:00. We can offer a Home-based Program via telephone for those that are unable or do not wish to join a Group setting. We also offer a 12-week home-based exercise program for Heart Failure patients using the Heart Failure Reach manual. Patients are expected to attend one of our venues at week 1 for an initial assessment, are then followed up bi-weekly by telephone and then need to attend a final discharge assessment at week 12.
Interpretation and translation services can be arranged by staff, for users of this service.

Referral Procedure:
|
To use this service, you will need to be referred by a health professional and meet the referral criteria, this is usually done by the Cardiac Centre, or Cardiac Specialist Nurse treating you. We do not accept self-referrals. The service will then contact you to make an appointment or explain what will happen next.

Telephone clinics and Group Sessions in a variety of venues around the County.

Parking and Transport:
|
There is free onsite parking at Aylsham, Swaffham, Norwich and Long Stratton, a small charge is required for the Fakenham venue, however free parking is available nearby.
|
Access and Facilities:
|
Main entrance at all venues well signposted and wheelchair accessible. Wheelchair accessible toilet facilities. All Venues are on the ground floor or lifts available.

We provide an adult service only.

The Patient Advice and Liaison Service (PALS) can listen to your issues, concerns, compliments or suggestions, resolve problems where possible, and feed back to services to aid service development.
