The service provides a link between the acute hospital setting and community services, working closely with the JPUH patient flow team.
If pressure relieving equipment is required, this can be provided upon completion of an individual patients assessment. Hospital beds/mattresses for patients / palliative patients who meet the criteria for specialist equipment to aid discharge.

Community Liaison will liase with Distirct nurses for patients being discharged home who require ongoing care, medication i.e. Insulin, Syringe drivers, and palliative patients.

Fast track continuing healthcare assessments for palliative patients requiring rapid package of care, if meets criteria.

How Service is Delivered: Within secondary care.
Opening Times: Monday to Friday, 8.45 a.m - 3.15 p.m.
East Coast Community Access is available 8.00 a.m - 8.00 p.m.

Access and Facilities: Main entrance, lift and toilet facilities are well signposted and wheelchair accessible.
Special Educational Needs Accessible: Children are not cared for within the Beccles Hospital environment. Young people of 18+ are accepted at Beccles Hospital if appropriate. On receipt of referral for either inpatient or outpatient services, the hospital or outpatient service will review the referral for indications of SEND requirements.

Where necessary, Beccles Hospital will liaise with the appropriate LA if the service user has SEND to identify the appropriate level of care and support required.

The Hospital will liaise with the young person and, if required, their families to ensure the service user fully understands the service they have been referred to, any concerns about equipment that may be used to provide their treatment and to understand any personal behaviours that may require consideration when providing treatment (noise sensitivity, routine etc).

The Norfolk County Council special educational needs and disability (SEND) Local Offer website offers a wide range of advice and support for families. The SEND Local Offer has two main purposes:
* To provide up-to-date information about SEND provision
* Allow the local authority to actively involve children and young people, their parents or carers and any health professionals in the development and review of the SEND Local Offer

Special Educational Needs Transition: As they approach their 18th birthday, if the young person is currently receiving healthcare, the provider of that healthcare will submit a formal referral to the adult services (if appropriate).
The provider will share basic care information to enable the adult service to appropriately triage the care and arrange the appropriate appointments.
The receiving service will liaise with the NCC local SEND team to ensure the service user continues to receive the right care, by the right professional, in the right location for the right duration without detrimental impact to that service user’s health and wellbeing.
Compliments / Complaints: The Patient Advice and Liaison Service (PALS) can listen to your issues, concerns, compliments or suggestions, resolve problems where possible and feed back to services to aid service development.
