The service helps patients with heart failure to look after their long term condition. The service also checks patients while they are having treatment by telephone, clinics or home visits. The service makes sure everyone has a care plan that is right for them.

Interpretation and translation services can be arranged by staff, for users of this service.

Home visits are provided by this service.

An information booklet for patients is available on request.

Referral Procedure: To use this service, you will need to be referred by a health professional. This means you must see your doctor or nurse will send your details to a service. Your doctor or nurse will refer you to the service that is the most suitable for your health needs. The service will then contact you to make an appointment or explain what will happen next.
How Service is Delivered: The service is delivered by home visits, telephone follow up and inreach into secondary care / community hospitals.
Opening Times: Monday to Friday, 8.30 a.m – 4.30 p.m
Messages can be left on answerphone or mobile phone.
Emails will be dealt with on return to practice.
Parking and Transport: Currently all patients are seen at home
Special Educational Needs Accessible: Communications team, Norfolk Community Health and Care.
Compliments / Complaints: The Patient Advice and Liaison Service (PALS) can listen to your issues, concerns, compliments or suggestions, resolve problems where possible, and feed back to services to aid service development.
