The Community Matrons and Case Managers are Nurses who work directly with patients who have unpredictable health needs particularly connected to long term conditions, such as respiratory or cardiac diseases that may cause frequent but unnecessary hospital admissions. This service is offered within patients own homes / care homes. The Community Matrons service is provided as part of the Community nursing and therapy teams Interpretation and translation services can be arranged by staff, for users of this service. Home visits are provided by this service.

Compliments / Complaints:	The Patient Advice and Liaison Service (PALS) can listen to your issues, concerns, compliments or suggestions, resolve problems where possible, and feed back to services to aid service development.
