The Children's Short Breaks Home Nursing Team provides short breaks for Norfolk families (except those in Gt. Yarmouth and Waveney) caring for children with chronic/complex health/nursing care needs. We aim to provide short term packages of short breaks care in the family home to families who are in need of extra support. A nurse or healthcare assistant will care for the child enabling the parent/main carer to have a break from the caring role. Continuing Care support is available to support those children/young people with highly complex healthcare needs.

Interpretation and translation services can be arranged by staff, for users of this service.

Home visits are provided by this service.

How Service is Delivered: At home
Opening Times: The team coordinators can be contacted Monday to Friday 9 a.m. to 4 p.m.
An answerphone is available.

Short Breaks are available throughout a 24 hr period but has to be booked with the coordinators in advance.
Special Educational Needs Accessible: Criteria for access are patients with complex conditions, medical and technical needs with / without a learning disability. The Co-ordinator reviews the care packages regularly and package of care hours are determined with the family using a care domain level of need approach. Packages are generally of 3-4 hours duration each week and can be flexible morning, afternoon, evening or twilight. The service is provided all year round except Bank Holidays and tailored to the family's request, where capacity allows.
Special Educational Needs Contact: A phone call to the Co-ordinator by the professional referrer, followed by a written referral. The Co-ordinator will ask the referrer to gain consent from the family. The Co-ordinator will contact the family within 2-3 weeks to discuss further and arrange a home visit to assess.

Special Educational Needs Transition: All short breaks packages are reviewed 6 monthly so the Co-ordinator will determine with the family the appropriate adult team to refer the young adult on to if respite support is still needed in the home or outside the home type short breaks.

Home respite is open to all patients aged 0-18 years with a Norfolk GP (not including Great Yarmouth and Waveney area).
Compliments / Complaints: The Patient Advice and Liaison Service (PALS) can listen to your issues, concerns, compliments or suggestions, resolve problems where possible, and feed back to services to aid service development.
