If you have recently had a stroke and stayed in hospital, this service may be able to help. They can offer rehabilitation to help with moving and talking.
Age Range:	16 and over

Referral Procedure:	To use this service, you will need to be referred by a health professional from the hospital. This means you must have stayed in hospital after having a stroke. The Doctor will refer you to the service that is the most suitable for your health needs. The Doctor might refer you to this service or a different service. The service will then contact you to make an appointment or explain what will happen next.
How Service is Delivered:	Community Service (ESD) is provided either to patients in their own homes or to in-patients on Beech Ward at Norwich Community Hospital.
Parking and Transport:	Onsite parking with parking spaces for disabled drivers, wheelchair access. Accessible by public and community transport.
Access and Facilities:	Main entrance and toilet facilities are well signposted and wheelchair accessible. Loop hearing aid system available.
Compliments / Complaints:	The Patient Advice and Liaison Service (PALS) can listen to your issues, concerns, compliments or suggestions, resolve problems where possible, and feed back to services to aid service development.
