The Colman Centre for Specialist Rehabilitation offers an outpatient service for patients needing specialist rehabilitation, this will be predominantly clinic based. The service provides multidisciplinary individually designed rehabilitation programmes for patients with highly complex health needs including acquired brain injury. These programmes aim to maximise the patient's potential in functional ability and quality of life, by working with patients and their families to identify the most beneficial goals for recovery. The team works closely with other services including local community teams and the inpatient unit based at the Colman Hospital.
Some sessions are being offered virtually if this is appropriate.
Home visits can be provided by this service based on assessment of need.
Interpretation and translation services can be arranged by staff, for users of this service.

To use this service, you will need to be referred by a health professional. They will send your details to the service. The service will then contact you to make an appointment or explain what will happen next

How Service is Delivered:
|
Predominantly clinics some of which are virtual clinics and groups based at The Colman Centre. Home visits and community visits can also be provided if indicated, based on assessment of need.

Opening Times:

Reception is open Monday to Friday 9.00 a.m - 4:30 p.m
An answerphone is available and messages will be picked up during office hours Monday to Friday.

Limited onsite parking and disabled spaces available.
Site is accessible by public and community transport.
Permit parking in place; check with the staff who will support you with this.

Main entrance and toilet facilities are well signposted and wheelchair accessible

The Patient Advice and Liaison Service (PALS) can listen to your issues, concerns, compliments or suggestions, resolve problems where possible, and feed back to services to aid service development.
