Wymondham Health Centre is the base for community health personnel. They work with the Wymondham and Humbleyard GPs and their staff, Social Services staff. Services are provided by a number of health professionals who work together for all members of the community. Wymondham Health Centre is a multi-agency hub, integrated with social services, including the South duty team, night owls and swifts. The hub provides centralised allocation for community nursing and therapy and the continence service for all South Norfolk.

* Community Nurses (Nursing needs assessments)
* District Nurses * Dietician
* Midwives
* Occupational Therapists
* Podiatrists
* Physiotherapists
* Rapid response team
* Speech and Language Therapist adults
* Service Leaders
* Volunteers Co-ordinator

Home visits are provided by this service (only if housebound).

Interpretation and translation services can be arranged by staff, for users of this service.

Referral Procedure: Referral via self-referral and health professionals
How Service is Delivered: Visits to housebound patients and clinic appointment
Opening Times: Monday to Friday, 8.30 a.m. - 4.30 p.m.
Single Point of Contact, 8 a.m. - 7 p.m. (telephone calls only)
Parking and Transport: Limited onsite parking. Pay and display car park at Central Hall. Health Centre accessible by public and community transport.
Access and Facilities: Wheelchair access and disabled toilet facility.
Special Educational Needs Accessible: NCH&C are committed to providing services that are easily accessible to all service users. If a service user may have difficulty in accessing our services for any reason it would be helpful if this information is included at the time of referral so that we can make sure that they receive the support that they need.
Compliments / Complaints: The Patient Advice and Liaison Service (PALS) can listen to your issues, concerns, compliments or suggestions, resolve problems where possible, and feed back to services to aid service development.
