The Wheelchair Assessment Centre provides an assessment of an individual's wheelchair and postural needs. Strict eligibility criteria applies for referral to the wheelchair service. Individuals who can use this service are from around age 3 to elderly. The service user must have a permanent medical and/or physical problem resulting in long term mobility issues.
Patients must be registered with GPs in Norwich, Broadland, North Norfolk, South Norfolk and West Norfolk to use this service.
Interpretation and translation services can be arranged by staff, for users of this service.
Home visits are provided by this service in exceptional circumstances only.

Referral Procedure:
|
To use this service, you will need to be referred by a health professional. They will send your details to the Wheelchair Service. The service will then contact you to make an appointment or explain what will happen next. If you are not currently being seen by a health professional, your GP can make the referral. Patients / advocates can refer if patient already registered with service.

How Service is Delivered:
|
Our service is delivered via clinics at our base at Julian Hospital, Bowthorpe Road, Norwich.
Some clinics are run from our West Norfolk base at Park View Resource Centre, Birch Tree Close, London Road, King's Lynn PE30 5QD.
Video assessments are provided on the NHS Attend Anywhere platform where appropriate.

The Wheelchair Assessment Centre is based in the Re-enablement Services Centre on Bowthorpe Road and is Monday to Friday, 8 a.m - 4 p.m. (excluding bank holidays)

Onsite parking. Accessible by public transport.

Main entrance well signposted and wheelchair accessible.
Wheelchair accessible toilet.

Our unit is fully accessible for all patients, there is a small play area in reception.

The service caters for patients from 3 years old to old age.

Compliments / Complaints:
|
The Patient Advice and Liaison Service (PALS) can listen to your issues, concerns, compliments or suggestions, resolve problems where possible, and feed back to services to aid service development.
