Referral Procedures (Heron):	To self-refer into this service, complete the <a href="https://www.norfolk.gov.uk/care-support-and-health/health-and-wellbeing/adults-health/weight-management" target="_blank">questionnaire</a> and ring the service with your answers on 01773 306 856. <br /> You will be assessed by a health care provider, who will determine if you are eligible and suitable for the NSWR programme. If eligible, they will then discuss the best option (group or online support) for you, and will offer a choice of an e-voucher or postal voucher for 3 months free Slimming World membership. <br /> <b>For Group Support:</b> <ul><li>If an e-voucher is chosen, a voucher code will be automatically sent via email to the Slimming World Consultant of the group the patient has chosen to attend.</li> <li>When the patient attends the chosen group, the e-voucher or postal voucher will be activated by the Slimming World Consultant.</li> <li>If a postal voucher is chosen, the patient is posted a pack by Slimming World containing a voucher.
