Heron notes:
The Looked After Children Service provides an integrated and coordinated approach to assessment and care planning for Looked After Children and Unaccompanied Asylum Seeking Children (UASC). Children and young people (birth up to 18th birthday) in the NHS Norfolk area who are in the care of the Norfolk County Council or placed in the County, by virtue of being 'looked after' by the local authority. <br /><br /> The Looked After Children service aims to promote the current and future health of all Looked After Children and UASC by ensuring that they have access to timely and appropriate holistic health assessments and the development of a health care plan that identifies health needs and specifies how these needs will be met, including support to access specialist services. <br /><br /> All children coming under the care of the local authority or those children placed into the Norfolk area will, by request, receive an initial health assessment and development of a health care plan within 20 working days of the notification of a child becoming 'Looked After'. <br /><br /> The initial assessment is completed in a clinic appointment by a medical practitioner. Thereafter, for children and young people over five years of age, will receive an annual health review by a Looked After Children's nurse at an appropriate venue. Children under the age of five years old will be seen by their health visitor service. <br /><br /> Interpretation and translation services can be arranged by staff, for users of this service.
