Heron notes:
The service provides care, advice, support, education and medication management to people who have been diagnosed with heart failure. Confirmed by Echocardiogram. Care is offered to those who are house bound as well as those who are able to attend a clinic. The heart failure service works closely with all of the other community health and social services.<br /> <br /> Home visits are provided by this service.<br /> <br /> Interpretation and translation services can be arranged by staff, for users of this service.
