Heron notes:
The Community Neurology Team provides tailored intervention to people within West Norfolk with complex needs, as a result of neurological conditions. As a specialist multi-disciplinary team we aim to visit people in their own environment, assessing the physical, emotional and psychological needs of the person using functional and standardised assessments. The service provides assessment, treatment, maintenance, practical advice and equipment to ensure these adults with a neurological condition and their family and carers are able to live and work to their maximum ability. The specialist team is also able to facilitate the patient's pathway from acute care to their home, liaising and signposting to other agencies as required. We offer a service to patients who have a new neurological diagnosis or have a progressive/on-going or complex diagnosis resulting in new needs.<br /> <br /> Interpretation and translation services can be arranged by staff, for users of this service.<br /> <br /> Home visits are provided by this service.
